Administration of Medicine
The program staff will administer prescribed inhalers, epi-pens, and non-prescription topical medication and EMERGENCY oral medications (i.e. Benadryl) with an authorization form that must be signed by a parent and doctor. Non-prescription medication will also be administered by authorized program staff and non-prescription still require an authorization form that must be signed by a parent and doctor. This form is available in the director's office. The form includes: Child's name, address and birth date; drug name and dosage; method of administration and time to be administered; side effects; and prescriber's name and address. All medication will be locked in the program director's office and will be labeled with the child's name, required dosage, date of birth and any other necessary information. Medications must be in their original container and must be clearly labeled. Other than emergency medication, no first dose of medication will be administered by Klub Kid, Inc. staff members.
The parent is required to bring the prescription with the doctors consent form to the program director on the day the medication is to be administered. Parents are not to leave medication in the child's cubby, lunch box or coat pocket. Any medication found that way will be discarded.
Staff will keep an accurate log of all medication that has been administered. Included in the documentation are: the date, time and dose that medication was administered along with the signature of the administering staff member and any additional comments.
Parents will be notified when/if a child has been administered any medication. Staff is trained in the administration of medication. At no time is an untrained staff allowed to administer medications.
Children with the following symptoms cannot be allowed to remain in the school for their safety as well as the other children: Fever, vomiting, diarrhea, profuse discharge from the eyes and nose, rashes of unknown origins, head lice, or the presence of a contagious disease. See sickness chart.
A child may be sent home if he/she shows symptoms of illness during the day. In such cases, he/she will be isolated from the other children and the parent will be contacted to pick up the child as soon as possible. The child will be supervised at all times during isolation until a parent or authorized individual comes to pick up the child.
In cases of accidental injury, we will make an immediate effort to contact the parents. Until the arrival of the parent, the Director/staff member will be in charge and make decisions concerning the care of your child. You will be expected to assume the responsibility for any incurred expense.
We are not equipped to care for ill children, so please be certain to have a plan for the possibility that your child might become ill at school.
The nurse consultant makes regular visits to check on records, meet with the staff, note health and safety conditions, and observe the children. If parents have concerns regarding their child's general health, growth and/or development, the nurse is available for consultation.
If the child is well enough to come to school, she/he should be well enough to participate in outdoor activities.
Child Abuse and/or Neglect Policy: Should we have any reason to suspect any evidence of Child Abuse and/or neglect, we are required by law to report it to the Department of Children and Families at 1-800-842-2288. We are mandated reporters and are required to report any suspicion to the DCF or a law enforcement agency within 12 hours.
