Mandatory Forms
- Medical permission forms must be completed and returned to Klub Kid, Inc. BEFORE your child starts school.
- We must have an up-to-date record of your child's immunizations prior to his/her starting school.
- Evidence of physical exams is required to be current within 12 months. During the year, you will receive a reminder when an update is due.
- A family history, financial contract agreement, emergency file card, permission slip, and registration form need to be completed prior to beginning school.
Scheduled Pick Up Times
If a child has not been picked up within 15 minutes of scheduled pick up, the following steps will be taken
- Attempt to reach parent(s) at work.
- Attempt to reach parent(s) at home and on cell phone.
- If parents cannot be contacted, the staff will call the individuals authorized on your emergency card to pick up your child.
- If neither individual with parental pick-up authorization can be reached, the staff will repeat the phoning process.
- If the persons authorized by the family to pick up their child cannot be contacted after 45 minutes of scheduled pick up, the staff will report the situation to DCF and the Glastonbury Police Department.
- Two staff people that the child is familiar with will remain with the child at all times.
Accident Reports
If an accident involving your child occurs during the day you will be notified of the accident, and the teacher will complete an accident notification form. This form will be given to you when you pick your child up. Please sign the form and return it to the Center the next day for proper documentation.
Meeting/Notices
- There are a number of parent functions that will be held during the year. Parents are encouraged to participate as well as suggest ideas and activities.
- All changes to the above and the other Klub Kid, Inc. policies and other necessary information will be posted on the bulletin boards near the sign-in sheets. Please read them daily.
- Teachers welcome the opportunity to meet with you at any time to discuss your child. Teachers will meet with you in January and in May for parent/teacher conferences.
Transportation
- Parents should accompany their children to their teacher. We cannot be responsible for children who are dropped off at the front or side doors. When parents are present at Klub Kid, Inc., they are responsible for their children.
- Parents are responsible for recording arrival and departure times on sign-in/out sheets posted in the kitchen.
- Children will only be permitted to go home with their parents or individuals they have authorized on the Enrollment form. There are permission forms to sign if your child is to be picked up by someone other than those indicated on the enrollment form.
- Transportation is available to and from all Glastonbury schools in one of our mini school buses. Our buses are registered in the State of CT and have passed required DMV inspections. Our bus drivers have met all physical and educational requirements to be licensed by the State of CT to transport children to and from school and on field trips.
- Consent for school pick up and/or drop off must be indicated on the Program Enrollment Form.
- Consent for Field Trip transportation must be indicated on the Enrollment Application and on a Field Trip Transportation Consent form for each trip.
Lunch
- Any utensils needed for lunch or snack will also need to be provided by the parents each day.
- Morning and afternoon snack and drink is provided for each child by the school. All snacks will provide nutrition from at least two food groups and the menu will be posted one week in advance. Please notify the teacher if your child is allergic to any foods.
- The parent must provide lunches and drinks for any child who will be in attendance during lunch time M-F 12pm-1pm. Microwaves are available for warming and refrigerators are located in the kitchen.
Clothing
- Comfortable clothing appropriate for current weather conditions should be worn. Rubber soled shoes are best for children to wear (cold or warm season). We do go outside every day, so please have the appropriate clothes available for your child.
- An extra set of clothing (including socks and underwear) is a MUST and should be left marked in your child's cubby.
Web Camera
The program is equipped with a web camera in each classroom and outdoors. The web camera is a unique tool which allows the parents and authorized caretakers to feel connected with their child at all times. Upon enrollment the parents or caretakers will receive a user name and password to log onto the appropriate web cam through a secure site. To log on, the parent must go to www.klubkidinc.com and click on web cam. At that point they will be able to enter their username and password. The program director will be responsible for providing the appropriate family the access information. Upon termination from the program, the program director will promptly remove their access to the web cam. All parents must sign a web camera consent form upon enrollment.
